KABUL, Afghanistan (AP) ? U.S. Defense Secretary Leon Panetta, who has flown to Afghanistan for talks with President Hamid Karzai, says President Barack Obama will decide in the next few weeks how many U.S. troops will after the combat mission ends there.

Panetta arrived Wednesday on an unannounced visit to Kabul to consult with top military commanders as well as confer with Karzai.

The visit comes at a difficult juncture in the Western coalition's efforts to shift more security responsibilities to Afghan forces so the combat mission can end in December 2014.

While security has generally improved lately, the Afghan government's ability to effectively govern and to root out corruption is in great doubt. Peace talks with the Taliban are on a back burner.

Speaking to reporters earlier Wednesday in Kuwait, Panetta did not reveal what options Obama is considering, but officials have said he may settle on a figure between 6,000 and 10,000 troops. There currently are about 66,000 U.S. troops in Afghanistan.

The post-2014 mission is expected to focus on counterterrorism and advising Afghan security forces.

Before flying to Afghanistan, Panetta spoke to about 100 U.S. service members inside an aircraft hangar at a desert base west of Kuwait City. He thanked them for their service and emphasized that the U.S. is winding down its involvement in lengthy wars.

Rock legends to perform at storm benefit

Paul McCartney jamming with Dave Grohl and Eddie Vedder duetting with Roger Waters on a Pink Floyd song are just two of the acts in store at a benefit concert on Wednesday for victims of Superstorm Sandy, producers say.?

The "12-12-12" concert at New York's Madison Square Garden features a who's who of rock and pop, including The Rolling Stones, Alicia Keys, Chris Martin, Billy Joel, Bruce Springsteen, Eric Clapton, Kanye West and Bon Jovi.?

Producer John Sykes said the fundraiser would feature "the greatest lineup of legends ever assembled on a stage."

He said Waters, McCartney and Chris Martin of Coldplay "are reaching out to other legends to join them on stage and create once-in-a-lifetime moments."

Sykes said $32 million has already been raised from ticket sales and sponsorships. With the concert's potential to reach 2 billion people through broadcast and digital platforms, organizers are hoping to raise tens of millions more.

To help with the fundraising, celebrities such as Leonardo DiCaprio, Kristen Stewart, Jake Gyllenhaal, Chelsea Clinton and Billy Crystal will take part in a telethon during the concert, which starts at 7:30 pm ET and is expected to last between four and five hours.

The "12-12-12" concert will be broadcast live on television, radio, movie theaters, on Facebook and iHeartRadio and streamed on digital billboards in New York's Times Square, London and Paris.

More than 130 people were killed when Sandy pummeled the east coast of the United States in October. Thousands more were left homeless as the storm tore through areas of New York, New Jersey and Connecticut, causing billions of dollars in damage.

Sykes said that personal stories of neighborhoods and people severely affected by Sandy will be showcased during the concert. Sykes was involved with "The Concert for New York City" after the September 11, 2001 attacks, which raised more than $30 million for charity.

He said technological advances over the past decade have exponentially changed the reach of fundraising.

"We have both traditional and new media behind us in a way that we've never had before, and that is really going to be the 'x-factor' on how much money we can raise for the victims," Sykes said.

Donations raised from the one-night concert produced by Clear Channel Entertainment and The Weinstein Company, will go to the Robin Hood Relief Fund, which will provide money and materials to groups helping people hardest hit by the storm.

Telemedicine takes root in the Midwest | Scope Blog

Earlier this year, first-year medical student Bonnie Chien and classmates visited the Rosebud reservation in South Dakota as part of their Rural and American Indian Health Disparities class. In a series of posts on Scope, she chronicled her trip and how a lack of funding and resources forced physicians to take on numerous roles and do a little bit of everything from performing circumcisions to treating rheumatoid arthritis.

So I was interested to read an Atlantic post today about Avera Health Network, a long-distance critical-care center where doctors use two-way video conferencing to provide much needed support to rural hospitals in South Dakota, North Dakota, Minnesota, Iowa, Wyoming and Nebraska.

Averna Health Network?s four main services ? eConsult, eICU Care, eEmergency, and ePharm ? are designed to provide some relief for the 10 percent of physicians in the U.S.?who currently serve 25 percent of the nation?s population residing in rural areas. As writer Lindsay Abrams explains, the nonprofit allows local emergency medicine physicians to immediately connect with specialists who can consult in treating patients as well as help in monitoring them:

When the call comes in the middle of the night, with the push of a button ? mounted right on the ER?s wall ? the nurses on-duty are able to connect with ER doctors in Sioux Falls, who have been waiting, in their patient-less hospital, for their call.

?

Of course, physical hands are needed to carry out virtual orders, and real doctors and nurses are always on hand to provide that. But even IRL (in real life), crisis situations require someone at the head of the room, keeping tabs on everyone and calling the shots. In emergency situations, where every second counts, the long-distance physician is able to be in the room an average of 14 minutes sooner than the local doctor.

The doctors back at the hub spend their time monitoring ICU patients ? they have virtual access to 60 percent of the beds in South Dakota. Pharmacists are on-hand to review prescriptions, make sure doctors aren?t missing any allergies or medical history, and keep them abreast of the newest recommendations and standards of care.
